Add Associated States

Idea proposed in #61, add associated states of Marshall Islands, Micronesia, and Palau.

Much like territories and commonwealths, we could add an us.states.ASSOCIATED attribute. They would also live in the us.states namespace as their abbreviation. Thoughts?
